Enrollment form fields
Configure the title, colors, custom fields, and consent checkboxes customers see on your card's enrollment form.
The enrollment form is what a customer fills out before their card is added to Apple Wallet or Google Wallet — you control its title, colors, and fields.
What you can configure
- Title: the heading customers see at the top of the form.
- Colors: match the form to your card and brand.
- Custom fields: add extra fields beyond the basics, such as a birthday field.
- Consent checkboxes: add a consent line, for example for marketing messages.
What's always collected
Every enrollment collects what Waya needs to identify the customer and issue their card — their name and phone number. Staff use that phone number to look the customer up later from the Scan tab.
Editing the form
- Open your card's enrollment settings
From the dashboard, open the enrollment settings for the card you want to edit.
- Update title, colors, or fields
Change the title text, form colors, add or remove custom fields, and edit consent wording.
- Save
Save your changes. New enrollments use the updated form.
Keep the form short
Every extra field is one more thing standing between a customer and adding your card. A short form — name, phone number, and maybe one custom field — enrolls more people than a long one. Add custom fields when you have a real use for the answer, like a birthday you plan to acknowledge, rather than collecting information you won't act on.

Can I ask for a birthday on the enrollment form?
Yes, a birthday field is available as a custom field, though Waya does not automate birthday messages — you send those yourself.

You can add a consent checkbox to the form if you want customers to opt in explicitly.
